**This position is open to staff working at the Trust in the ‘ECT Family’ this includes current East Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.**

Are you an experienced clinician who would like to develop their knowledge and skills further? Are you ready to commit to a 3-year Masters Level apprenticeship at MMU?

 We are seeking an enthusiastic professional who is keen to join our team of community ACPs who will be on hand to support and guide you through your training.

The role you will perform once qualified includes supporting people in their own homes when they are in acute crisis and striving to avoid unnecessary hospital admissions, keeping people where they would prefer to be.

You will support the developing virtual wards and care for complex patients in their own home with the guidance from the consultants. And you will be involved in the strategic development of community services, using your research and QI skills.

Hours per week: 30.00

East Cheshire Trust would like to offer the successful candidate a place on the MMU ACP apprenticeship to start in January 2026. This is a 3 year course at Level 7.  The successful candidate will have a mentor and be supported to achieve their competencies and develop into a qualified ACP within the community.

The community ACP is responsible for assessing, diagnosing and treating patients in their place of home who are unwell and at high risk of admission to hospital. 

They utilise all 4 pillars of advanced practice; Clinical, Education, Research and Leadership to ensure they are providing the highest quality of care. 

They are a clinical leader within the care communities they serve and work with the wider MDT to develop services. 

They also work with the Consultants at the hospital, MDGH, to support patients on the virtual ward. Ensuring complex patients are supported to stay at home or have a supported early discharge.

  • Current registration with Professional Body NMC/HCPC (Nursing and Midwifery Council / Health and Care Professional Council)
  • First Level degree or relevant qualifications
  • Eligible for entry onto MSc programme
  • Recognised teaching/ assessing qualification e.g. ENB 998: City and Guilds 730(7) Teaching and assessing adults

  • ALERT course (Acute Life-threatening Events—Recognition and Treatment IRMER training

Knowledge
