Job overview

An exciting opportunity has arisen for a Band 4 Urology Cancer Support Worker - Personalised Stratified Follow Up (PSFU) at Warrington and Halton Teaching Hospitals NHS Foundation Trust

You will be responsible for monitoring appropriate patients via the My Medical Record portal who have finished their cancer treatment and are now on follow up. You will work closely with the Clinical Nurse Specialists ensuring that patients are followed up and have access to health well being advice and holistic needs assessments where appropriate

You will work closely with the Clinical Nurse Specialists and the Multi-Disciplinary Teams within the specialties as well as the rest of the Cancer Services Team.

The Cancer Management team is committed to the personal development of team members and there will be opportunities for development within this role.

Enthusiasm, commitment and flexibility are required for this role along with good customer service and excellent organisational and communication skills. Ability to work under pressure in a fast paced environment with excellent attention to detail is absolutely essential.

 

Main duties of the job

The post holder will be an integral part of the Urology Team. Working under the guidance of a registered health professional, this role supports people living with cancer who have been assessed as having noncomplex needs and who are able to self-manage their care needs. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.  The support worker will act as a keyworker, identifying a person’s holistic needs, coordinating a person’s care, navigating the health and social care system on their behalf and using a dedicated database (My Medical Record) to track patients through their follow up and surveillance ensuring that milestones are met.

 

 

The post holder will demonstrate excellent organisational skills, must be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity. A key aspect of the role involves daily and direct (e.g. face to face and telephone) communication with patients, relatives, carers, and other health and social care professionals. The information and nature of the communication required is sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity and for this reason post holder will have excellent communication skills and be willing to undertake on-going training and development.

Qualifications

Essential criteria
  • 5 GSCEs including Maths and English or equivalent
Desirable criteria
  • Educated to degree level

Experience

Essential criteria
  • • Relevant Health or Social Care experience • Efficient in the use of Microsoft Word for Windows, Excel, Power Point, Outlook and internet. • Advanced knowledge of medical terminology. • Understanding of person-centred care. • Experience of coaching/teaching patients and carers.
Desirable criteria
  • • Understanding of Cancer Support and its role • Knowledge of relevant cancer treatments, interventions and terminology.
