Job overview
Working alongside our existing Advanced Practitioners and experienced MDT, the post-holder would be responsible for providing a service across the acute frailty pathway. This role requires excellent organisational and interpersonal skills, alongside sound clinical knowledge, relevant experience and the ability to work at pace.
The acute frailty pathway gives advanced clinical practice high priority. Supporting people living with frailty to self-care and maintain what matters to them most are our key functions. We provide a truly integrated pathway, collaborating with our health and social care partners to improve patient care and communication across the whole system. The successful post holder will be expected to use their qualifications, skills and expertise to continue support and development within these areas.
This is a rotational system wide post, where you will be responsible for providing specialist clinical support to the whole frailty pathway; Frailty Assessment Unit, Emergency Frailty Department, ED in reach, Acute Frailty Same Day Emergency Care , supporting our Acute Frailty Virtual Ward and the Frailty inpatient bed base
Main duties of the job
This role is an exciting opportunity for you to be part of a new system wide model of care for our population living with frailty. You will be assessing patients with complex needs, undertaking physical examinations using appropriate use of investigations in order to reach a diagnosis. As part of the multiprofessional team, you will support in developing and contributing to the improvement of services. We are looking for someone who can demonstrate evidence of autonomy, clinical judgement who has excellent communication and decision making skills.

Experience
Essential criteria
- Registered AHP or first level nurse, with MSc Advanced Practice or equivalent
- Minimum 5 years post registration experience
- Extensive experience with area of clinical practice
- Line management experience within an advanced practice team / team management activity.
Desirable criteria
- Leadership Course
- Quality Improvement practitioner
- Experience of supervising trainee advanced practitioners
Qualifications
Essential criteria
- Full completed MSc. in Advanced Practice
- Registered Professional NMC, HCPC GPHc
- V300 independent non-medical prescriber
- Specialist clinical courses completed
Desirable criteria
- Leadership qualification
- Good understanding of the training and development needs of trainee advanced practitioners
Skills, Knowledge and Competencies
Essential criteria
- High complex reasoning and prioritisation skills, across clinical and staffing requirements with advanced practice in the department
- Highly experienced at disseminating and presenting complex information using both verbal and written disciplines across a broad range of clinical and non-clinical activities
- Request diagnostic imaging within scope of practice and interpret the findings acting accordingly
- Expert professional / clinical knowledge of Advanced Practice at local and national level
- Evidence of developing and implementing policies and protocols where identified within advanced practice
Desirable criteria
- Evidence of supporting and supervising trainee advanced practitioners
