Job overview

Being at home or in a community setting is the best place to continue with a person’s recovery once an illness requiring hospital care is over and reduces unnecessary time in hospital for people for whom a community or other setting is most appropriate.

Following a stay in hospital, most people can return home without any further support. However, for some people, they may need further support, ranging from a very short period, to support for the rest of their lives. This can include:

1)    A further period of rehabilitation and recuperation, with extra support for a period of days weeks, or sometimes months until they regain full health or reach their agreed maximum potential.

2)    Returning home with a care package of services, aids or adaptations made to their home.

3)    Added support, which is only available in a care home, which will last for a long period and often for the rest of their lives.

A safe, effective transfer from hospital helping people to return to living independently at the right time is a cornerstone of our health and social care system. At Warrington and Halton Hospitals Teaching Hospitals NHS Foundation Trust (WHH) and Warrington Borough Council (WBC), our Transfer of Care Hub has been on a programme of transformation taking integration beyond co-location by working together as an integrated team creating a whole system that aims to deliver tomorrow’s service today.

Main duties of the job

As a Transfer of Care Nurse, Physio or Occupational Therapist, you will be a valued member of the Transfer of Care Hub working with other health and social care colleagues, carers and families and other statutory and voluntary agencies to identify and plan the support people require to return to the community. Our team is solution focused placing the needs of the person at the heart of our decisions, ensuring they have the support they require to not only return to the community but also to live their lives well and independently.

Collaborative working and the ability to create and maintain effective working relationships with a range of health and social care colleagues both in the hospital and the community are an essential element of the role. Working within the Discharge to Assess model, you will place the needs of person at the heart of your assessment and the MDT’s decision-making. Ensuring risks are evaluated and managed with the support of your professional colleagues.

Detailed job description and main responsibilities

The Transfer of Care Hub is a multi-organisational Health and Social care team based within Warrington Hospital with a shared vision to ‘Support our patients to live and age well, independently at home for as long as they are able’. Our primary role is to ensure timely and safe assessment and discharges from Hospital to various community settings. The Integrated Discharge Team provides a 7-day service between the hours of 7am – 6pm each day. The post holder will be expected to work on a rotational basis.

Qualifications

Essential criteria
  • • A Degree or Post Graduate Qualification in Nursing, Occupational Therapy or Physiotherapy
  • • NMC or HCPC registration
Desirable criteria
  • • Clinical Diagnostics Qualification
  • • Practice Tutor/Mentor Qualification
  • • Leadership Qualification/Training

Physical skills e.g., use of tools, equipment, minute taking, advanced computer skills

Essential criteria
  • • IT skills including Microsoft word and excel
  • • Use of equipment relating to clinical observations
Desirable criteria
  • • Trained in WHHFT IT Systems (Lorenzo, E-Outcome) or similar EPR systems

Experience

Essential criteria
  • • Experience of working within a health or social care setting and have knowledge of discharge pathways/policies.
  • • Experience of MDT working to achieve patient-focused outcomes.
Desirable criteria
  • • Experience of Discharge Planning.
  • • Experience of Ward-Based Work.
  • • Experience of working within an Integrated Service.
  • • Experience of working with the public and ability to provide non-clinical advice to patients, carers and outside agencies.
