Job overview

This vacancy is open to applicants currently employed by Cornwall Partnership NHS Foundation Trust, Royal Cornwall Hospitals NHS Trust, or the Integrated Care Board for Cornwall and the Isles of Scilly

We are seeking a highly skilled and motivated Occupational Therapist to join our team, delivering assessment and treatment to patients within the community, including their own homes and care settings across East Cornwall. You will work closely with the Early Intervention Home First Team and the Community Rehabilitation Team to support patients’ independence, recovery, and wellbeing.

Key responsibilities include:

  • Providing specialist occupational therapy assessment and interventions tailored to patients’ individual needs.

  • Working collaboratively within a multidisciplinary team to ensure effective and timely care.

  • Supervising and supporting junior staff and contributing to their development within the service.

Main duties of the job

       To manage a defined caseload of patients with complex needs within the Community setting using evidence-based practice, Occupational Therapy process and patient-centered principles

·        To provide a high standard occupational therapy to the service, performing advanced assessments of patients including those with highly complex presentations, and from this formulating a diagnosis and treatment plan.

·        To operate as a practitioner being the first contact for soon requests to help both prevent hospital admission and facilitate early discharge.

·        To have an active role with the triage of new referrals coming into the teams.

·        To provide leadership and support for junior staff through supervision, training, and appraisal as well as leading with Occupational Therapy students

·        To participate in the planning and development of Occupational Therapy services.

·        To cover a 7-day Rota.

·         To participate in the planning and development of occupational services.

·        Model and deliver excellent evidence based clinical care, working as an autonomous Practitioner.

·      To support the community rehabilitation and re-ablement model in line with a ‘one workforce’ culture across services, settings, and sectors in the community and step-down intermediate care settings.

Education and Qualifications

Essential criteria
  • Professional Qualification - Degree or Diploma in Occupational Therapy.
  • Current Professional registration - HCPC
  • Post graduate qualification to diploma level or equivalent relevant experience or Evidence of professional knowledge acquired through specialist courses

Knowledge and Abilities

Desirable criteria
  • Good teaching ability/education experience providing training in a range of subjects on a regular basis

Skills and Aptitude

Essential criteria
  • High level of specialist assessment and treatment skills.

Experience

Essential criteria
  • Broad range of post graduate experience of medical, surgical, orthopedic, and neurological settings
  • A broad range of clinical experience in a variety of community and inpatient settings.
  • Extensive experience of working within a multidisciplinary team
  • Comprehensive experience of working as a Occupational Therapist with patients with complex conditions
