Speak to us about how we might be able to accommodate a flexible working arrangement whether that’s job share, part time or another flexible pattern. In addition, although the role advertised may have a ‘home’ office base indicated, we remain committed to supporting flexibility around workplace locations. If it works for the service, we will do our best to make it work for you.

Unless exempt, we encourage and support staff to be vaccinated as this remains the best line of defence against COVID-19.
Job overview
This job is currently only open to people who already work for the NHS in Cornwall or the Isles of Scilly.
We are committed to supporting our dedicated staff, so only current employees of the following organisations can apply:
- Cornwall Foundation Trust
- Royal Cornwall Hospitals NHS Trust
- Cornwall and Isles of Scilly Integrated Care Board
If you do not currently work for one of these NHS organisations, your application will not be considered at this time.
The role is similar to a Health Care Assistant in that you will be delivering great care and supporting reablement for people in mainly in their own homes. This is a developmental role, developing from Band 2 to Band 3.
You will be within the HomeFirst Carrick team, covering areas including Falmouth, Penryn, Truro and Perranporth. Travel costs for journeys during work time are supported and pool cars are also available for work use, although not guaranteed. Due to the rural location of Cornwall, public transport will not be sufficient to get to patient visits. Ability to get to visits independently is essential for this role, usually you would drive and have access to your own vehicle to achieve this.
You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.
You will be part of a dedicated team and will be provided with bespoke training to support your development in this role.
Main duties of the job
- The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting
- Assist the patient to achieve their reablement goals using a range of specific activities, primarily on an individual basis, but also in groups, without direct supervision. This may include activities for developing independence in daily living skills, e.g., supporting with personal care and promoting a return to normal function and activity as appropriate, support and encourage the appropriate use of aids/equipment and reinforce self-care wherever possible.
- The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital
- The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes
- The postholder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service.
- To report any concerns or issues to the manager in charge
- To make or receive telephones calls
Working for our organisation
We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.
We are a people organisation and people matter to us. As part of the team, you'll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.
Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.
We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.
Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
