Job overview

An exciting opportunity has arisen for an enthusiastic, innovative, flexible, and well-motivated Allied Health Professional (AHP) Support Worker to join our friendly East Integrated Rehabilitation Team at Launceston and Liskeard Hospitals.

We deliver occupational therapy and physiotherapy in the East Cornwall locality to individuals with a primary physical health condition in variety of ways:

Launceston and Liskeard Community Hospitals – providing rehabilitation within GP-led units alongside occupational therapists, physiotherapists, nurses, and AHP support workers.
Community Rehabilitation – supporting people in their own homes with conditions such as neurological disorders, orthopaedic recovery, falls, frailty, and long-term conditions.
Intermediate Care – delivering urgent rehabilitation in partnership with Home First and Early Intervention teams.
East Community Orthopaedic Service Team – working with patients before and after elective hip and knee replacement surgery.

Main duties of the job

In this rewarding role, you will:

✅ Undertake skilled AHP support work, including assessments and interventions to aid patients’ rehabilitation.
✅ Plan, implement, and evaluate personalised treatment interventions.
✅ Empower patients and carers to take an active approach to regaining and maintaining their independence.
✅ Support our occupational therapists and physiotherapists by carrying out timely reviews of rehabilitation care plans.
✅ Manage your own caseload with support from the wider team.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health.  We also provide specialist support to people with dementia or a learning disability.

Education and Qualifications

Essential criteria
  • GCSE English and Maths or equivalent
  • NVQ4 in Health & Social Care or equivalent qualification/experience
  • IELTS unless qualification taken in English

Experience

Essential criteria
  • Substantial previous experience working as a Therapy Support Worker in a similar setting with an understanding of their roles and respect for the other professionals within the team
  • Extensive experience of working in a range of therapy/ care settings eg hospital, community
  • Experience of working without direct supervision, including monitoring and reviewing the person’s progress

Skills, Aptitude, Knowledge and Abilities

Essential criteria
  • Able to observe and modify a treatment activity, demonstrating an understanding of the patient’s needs
  • Ability to manage own caseload
