We have an exciting opportunity for a caring and motivated Community Nurse within the Arbennek Community Nursing Team.

We are seeking a dynamic, forward-thinking professional who wants to help us deliver exceptional nursing care in the community. A commitment to team working and providing excellence in patient care is a must as are exceptional communication and people skills. An ability to prioritise and manage own daily workload is required in this role.

You will need to be able to demonstrate real enthusiasm for community nursing as well as a desire for on-going professional development.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. 

 

 

Main duties of the job

Responsible for the assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment, enabling them to enjoy maximum independence and optimum quality of life.

To work in partnership with patients and carers to offer support and advice and promote informed choice.

Responsible to support the role of the Community Nurse Team leader and other locality teams by working in a flexible way where need and service provision demands.

To work autonomously, providing effective, evidence based care in line with competency and training record and organisational policies, procedures and guidelines.
