Job overview

An exciting opportunity has arisen where we are seeking an experienced and forward‑thinking Enhanced Practitioner to join our Dementia and Older Peoples Psychiatric Liaison Service. The Enhanced Practitioner will work as a senior member of the team supporting older adults' experiencing mental health problems and people living with or suspected dementia, who are receiving care in acute hospital settings and community hospitals.
You should be able to demonstrate a clearly your continued professional development to maintain your ability to practice in an evidence based way.

This position provides the successful clinician with a rare opportunity to help test and inform the development of enhanced clinical roles within an evolving mental health service. With the expectation of undertaking the Enhanced Practitioner qualification.  Working alongside partner organisations your skills and tenacity will support the integration of existing care providers to enhance the timely delivery of care to our population in Cornwall.

Main duties of the job

The post holder will provide timely, expert mental health assessment, formulation, and intervention that promotes safety, dignity, and improved health outcomes.
Working across the four pillars of practice, the Enhanced Practitioner will bring enhanced clinical expertise to complex presentations, offering high‑quality assessment, risk management, and evidence‑based therapeutic approaches.  Providing visible clinical leadership within the liaison pathway, supporting colleagues through supervision, modelling best practice, and contributing to a positive and collaborative multidisciplinary culture.

Education and development form a key part of the role, including providing training, coaching, and clinical guidance to both mental health and acute hospital staff to improve understanding of dementia, delirium, and mental health needs in later life. You will also contribute to audit, quality improvement, and service development initiatives—ensuring that current research, national guidance, and emerging evidence underpin the care we deliver.
Working autonomously and across the four pillars of practice—clinical, leadership/management, education, and research—the post‑holder will deliver expert mental health input to support the safe, effective, and person‑centred care of individuals with complex cognitive, emotional, and behavioural presentations. The role involves supporting the development of the liaison pathway, offering clinical leadership, and working collaboratively across health and social care systems.
This is an excellent opportunity for a confident, compassionate practitioner who is committed to improving the experience and outcomes of older people in acute care. If you are motivated by high-quality, person‑centred mental health care and keen to influence practice across systems, we would be delighted to welcome you to our service.
You should be a competent non-medical prescriber or alternatively have a commitment to develop your skills by undertaking the module in due course.

Working for our organisation

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly.  We deliver community and hospital-based care to improve people’s physical and mental health.  We also provide specialist support to people with dementia or a learning disability.

We are a people organisation and people matter to us.  As part of the team, you’ll help support the health and wellbeing of the people who live and visit this beautiful part of the UK.

Over 5,000 people make up the Trust.  This includes doctors, nurses, therapists, plus admin and support staff.

We work in people’s homes, in community clinics and bases.  Some staff work from one of our 10 community hospitals.  Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here.  A third of people who live in Cornwall are supported by acute hospital services in Devon.  As a result, we also work closely with our partners in Devon.  In the summer, and during other holidays lots of people choose to visit the area.  This increase the numbers of people who use out services.
