At the Dementia and Older People’s Mental Health Liaison service, no day is the same. Our service is based within the Royal Cornwall Hospital, Truro, but we outreach to our 8 local community hospitals and also provide community visits to support our patients and carers when they leave hospital. As a band 6 practitioner you will be involved in staff supervision, Quality Improvement, audit, teaching, and development of our service.

The post of Psychiatric Liaison practitioner is a specialised mental health practitioner role specialising in older adult psychiatry liaison.
You will be required to have extensive knowledge and skills in caring for older people with mental health problems, risk assessment and think outside of the box to ensure safe delivery of care and discharge out of the acute setting.
The post holder will utilise these clinical skills in ensuring the psychiatric liaison team provide high quality psychosocial assessment care and treatment, facilitating a holistic approach to service delivery and discharge planning.
The term holistic in this context is of particular importance, as patient’s physical care needs in addition to psychological and social care needs must all be carefully considered in the acute general hospital setting.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
