Providing specialist advice and practical support to patients that are homeless or in housing need and in hospital. 

Offering advice and support to ward staff to facilitate a timely and safe discharge to homeless patients.

Main duties of the job

Visiting patients on the ward to understand their housing needs. 

Ensuring the Trusts met their duty to refer under the Homelessness Reduction Act. 

Liaising with community partners and referring to accommodation providers to support arranging a safe discharge.
