Job overview

We are looking for an enthusiastic, highly motivated band 4 Community Nurse Associate  to join the Barrow Town  District nursing team for a period of 12 months from March  2025 - February  2026 to cover maternity leave.  
The post is a temporary fixed term   position, to cover  the maternity leave period.
The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.
The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programs of care for patients and carers. The district nurses work a 24hr service and regular weekends, evening shifts and occasional nights will be part of your rota.
You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting.

The post holder will work as a registrant within Community Services, accountable for providing evidence based, holistic nursing care of patients.
The service covers a 24 hour period 365  days a year. It is the expectation of this post that the post holder will work weekends, bank holidays, evenings and night shifts as required.
 

Working for our organisation

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre. 

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services. 

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses. 

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay. 

Detailed job description and main responsibilities

The post holder will be required to work as part of the integrated team across community settings such as patients’ homes, residential and nursing homes. Flexible working will be required to meet the needs of the patients group and the service as it continues to develop. Whilst working with other health care professionals the post holder will develop the ethos of integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
