Job overview
The Acute Frailty Team are seeking to recruit a highly motivated individual as we continue to develop our services. The ideal candidate will not only have an interest in improving patient care within frailty, but also actively contribute to be the development of education and training. This is fundamental to the role as the upskilling of the local workforce builds awareness of frailty, leading to improved identification and management for this patient group.
Delivery of person-centred care, screening for frailty and comprehensive geriatric assessment (CGA) are just some of the interventions that the successful candidate will be involved in across the frailty pathway. You will be supported to develop your clinical practice and leadership attributes, working towards specialised frailty competencies within clinical practice.
Main duties of the job
The role aims to facilitate early identification with focused interventions for all adults with frailty, presenting to our Emergency Department. Management of patients with frailty is therefore improved and the developing frailty pathway aims to follow these patients through the journey, seeking to improve associated adverse outcomes. We work closely with our community services and partners and harness a sense of collaborative working. Expanding frailty services beyond traditional models of working requires an ability to move and grow alongside this service and offers an exciting opportunity to join the team.
Working for our organisation
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our Community Service for adults are provided in people's homes ,community centres, clinics, GP Practices, community hospitals. Our aim is to work with people to help them remain independent , improve their health and manage their conditions through high quality care , advice and support.
