Job overview
We are seeking proactive and committed clinical support workers to join an innovative new programme in Barrow, designed to deliver intensive support for patients in the community and who are at high risk of hospital re-admissions.
You will join a newly established dynamic multidisciplinary team in supporting the delivery of holistic treatment plans through multi-agency coordination. The case management service brings together professionals from a range of services including (but not limited to) medicine, nursing, pharmacy, social care, therapy, voluntary sector, and community services.
Main duties of the job
Working as a clinical support worker you be clinically supported and supervised to undertake delegated nursing procedures, interventions or actions which have been identified by the MDT team as necessary to support clinical decision making and delivery of the holistic plan of care. Working independently within the patients own home, you will support and empower patients to achieve short term goals, promoting their wellbeing and support in health education to enabling independence and self management of their long term health conditions in the community. You will work within defined parameters to undertake technical and clinical care. You will effectively communicate and update the MDT with patient outcomes, current treatment plans and reviews. This rewarding and developing role offers opportunities for professional growth and the chance to make a real difference in the lives of some of the most vulnerable and complex patients living in Barrow community.
The service aims to reduce emergency admissions, promote independent living, and enhance coordinated care through a multi-disciplinary, holistic approach. The urgency is driven by the need to alleviate hospital pressures and improve patient outcomes.
Working for our organisation
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Education and qualifications
Essential criteria
- NVQ Level 3 in health and social care or relevant experience
Desirable criteria
- European computer driving licence/equivalent IT qualification. Functional literacy and numeracy skills at Level 2/GCSE English and Maths
Experience
Essential criteria
- Experience of working in a team
- Experience of providing care to adults
- Experience using MS Office and other IT applications
Desirable criteria
- Experience of working within a health or social care service
