Assistant Cardiac Rehabilitation Practitioner, Band 4 up to 30hrs a week across site -12 month Fixed Term

The  NCIC east cardiac rehabilitation team provide services to patients from the Carlisle and Eden area. The team currently provide Cardiac rehab at 3 sites – Cumberland Infirmary , Penrith Hospital and West Cumberland Hospital . Our main aim is to promote and facilitate an optimum return to function for all patients referred to the service including promotion of self-management/ referral for phase 4 rehab on discharge from the service.

The post holders will work as part of the Cardiac rehabilitation service at Cumberland Infirmary, Carlisle, and West Cumberland Hospital which is jointly run with specialist cardiac rehab nurses and physiotherapists from both sites. Work will involve maintaining a high standard of patient care by planning, undertaking and evaluating clinical interventions of management within a framework established by the Clinical lead.

Main duties of the job

The post holder will be responsible for a manageable caseload within the Cardiac rehab team, carrying out assessment and treatment for patients with a wide range of cardiac conditions. They will also take an active role in promoting self-management and third sector services once patients have finished their cardiac rehab programme.

The post holder will independently organise and manage the flow of patients through phase 3 of the cardiac rehabilitation programmes.

The post-holder will undertake administrative duties including searching for investigation results, appointment booking and photocopying.

Essential criteria
  • Foundation degree or Level 5 qualification in health and social care/ sports and exercise eg DipHE, HND etc
  • BACPR phase 4 certification or equivalent

Knowledge & Experience

  • Ability to plan and deliver a physical activity programme for cardiac patients
