Job overview

Discharge Navigator, full and part time available

Work within the environment to support the multi-disciplinary team, the patient and their carers in preparation for the patient to leave hospital. Working to support ward areas to ensure that the patient’s discharge is a smooth, safe transition from hospital to home or into community provision, in line with the Transfer of Care (discharges) from Acute and Community Settings policy. Supporting patient flow and contributing towards reducing the patient’s length of stay through the facilitation of effective integrated discharge planning.

Main duties of the job

The Transfer of Care Hub (ToCH) is a Service which has been developed to implement an expert complex multi-disciplinary and inter-disciplinary discharge team, working in a seamless and integrated way across partner organisations, both health and social care. The ToCH will proactively ‘pull’, and case manage the needs of a range of people with complex discharge needs on discharge pathways 1-3 and progress these people safely to transfer/discharge via an appropriate pathway

The Trust is committed to safeguarding adults and children therefore if the post is subject to the Rehabilitation of Offenders Act (ROA) 1974 (Exceptions) Order 1975 (as amended) the successful candidate(s) will be subject to a criminal records check with the Disclosure and Barring Service (DBS). The cost of the DBS must be met by the successful candidate(s) through salary deduction. DBS charges are as follows if applicable to the post:

 

Standard Check: £18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.

Enhanced Check: £38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following 3 months.

Qualifications

Essential criteria
  • • Evidence of a good standard of general education/GCSE English and Maths or equivalent.
  • • NVQ 4 in Care or equivalent experience or NVQ in Admin or equivalent experience
Desirable criteria
  • • Customer service skills

Knowledge

Essential criteria
  • • Evidence of knowledge of Hospital environment and pathways
  • • Accepts responsibility for own workload
  • • Seeks and uses support appropriately
  • • Knowledge of medical terminology, anatomical and physiological terms; acquired through work-based training
  • • Basic knowledge of Care Act 2014

  • • Recent work within a Hospital setting.
