Job overview
Cancer Care Coordinator Band 4 - 37.5 hours available - Urology Service - Cumberland Infirmary supporting clinical activity at West Cumberland hospital and Workington Hospital
You will provide effective, caring and compassionate services to patients working within a multi-disciplinary team (MDT) to deliver a high standard of care and support to patients with cancer. This will involve clinical interventions, with the support of the nursing team and co-ordinating care through from referral to the end of treatment. This will require regular contact with patients suffering from cancer. You will be expected to support and signpost patients face to face and over the telephone and ensure effective progress along their care pathway in liaison with other colleagues and departments in the hospital.
Main duties of the job
Duties include (but not limited to):
- Working as a team alongside service Cancer Care Coordinator and as a member of the Cancer Nursing team providing coordination of care and support for cancer patients, their carers and families
- Navigate the care pathway, as an advocate for the patient, including coordinating results and patient appointments and updating the clinical team as needed.
- Collaborate and communicate with the wider healthcare Team to ensure the provision of appropriate services to maximise the health and quality of life of cancer patients, improving the quality and efficiency of health care delivery.
- Act as a point of contact for patient and their carers and the wider health care team, signposting enquires to the appropriate department within hospital or community settings
- Actively participate in the provision of routine information and advice, for patients and their carers via the telephone, liaising with the CNS/AHP team as appropriate.
- Work closely with MDT coordinators and cancer trackers to ensure timely and efficient progress of patient care to ensure no patients breach cancer targets
- Identify opportunities for service improvement and implement these accordingly, in conjunction with the MDT.

Standard DBS check £21.50, Enhanced DBS check £49.50 + any admin costs.

Experience
Essential criteria
- Recent experience working with cancer patients or in cancer services
- Significant NHS experience or equivalent
- Experience working with a multi-professional team, and a medical or clinical team.
- Experience of direct patient contact including telephone and face to face contact.
