Job overview
North Cumbria stroke services are delighted to offer an exciting opportunity of a fixed term, part time 30 hours per week post, to be involved in implementing a new stroke discharge and support service. This role will be crucial to the efficient day to day delivery of the service
The community stroke service provides specialist assessment, rehabilitation and advice for patients, families and carers. Emphasis is placed on the provision of a holistic care package tailored to the individuals needs with the aim of developing maximum independence and hence improving quality of life after stroke.
The team provide specialist case management and therapy work to continue for up to 6 weeks post hospital discharge in peoples own homes, residential or nursing homes and liaising with other community services. They also complete reviews of patients needs at 6 months following their stroke to ensure patients have the support they need in life after stroke.
Main duties of the job

Secondments will be considered and the base can be negotiated at the interview stage.
You will provide a high standard of administration support to the clinical team with the following tasks;
- processing referrals into the service
- liaising by telephone or email with hospital wards, other departments, GP's, adult social care service, third sector organisations and patients and their carers.
- planning and booking of team appointments using electronic diaries
- writing discharge letters, completing referrals to other services, updating patient documentation, taking minutes in meetings, collating service data and audits.
- processing staff management tasks such as processing annual leave, sickness absence and appraisals on electronic system
- Competent use of IT systems such as excel spreadsheets, Microsoft Teams and electronic patient recording systems is essential.
Excellent communication and interpersonal skills are required as well as a positive approach to working in a team.
The role will involve some travelling across the North Cumbria region as well as utilising technology to offer services virtually

Qualifications
Essential criteria
- GCSE English
- NVQ 2 in Business Administration or Customer Service or equivalent experience
Knowledge and Experience
