Job overview
This is an exciting opportunity for someone experienced in working in community settings. The role involves both clinical work and service development to advance the home care practitioner and virtual ward services, which are vital for supporting patients at home and managing patient flow, especially during winter pressures. The post-holder will play a key role in developing these services in line with local and national strategies to improve patient care and outcomes. The role requires someone who can work effectively across different staff groups, show initiative, work independently, and perform well under pressure, particularly when patient flow is challenged. Main duties of the job
The post-holder will be a key senior leader within NCIC, responsible for driving high standards of clinical practice, professional development, and service improvement across the community collaborative. They will lead initiatives to embed evidence-based care, support education and research, and ensure alignment with the Trust’s clinical priorities and national strategies.
This pivotal role combines clinical and operational leadership, focusing on the development of the Home Care Practitioner service and Virtual Wards. The post-holder will contribute to designing and scaling virtual ward models that support safe patient flow, timely discharges, and winter resilience planning.
They will provide expert clinical guidance, foster a culture of continuous improvement, and champion equality, diversity, and inclusion. Working closely with clinical and managerial colleagues, they will lead practice development initiatives that enhance staff experience, promote multidisciplinary teamwork, and improve outcomes for patients.

Please note the cost of the DBS check is increasing from 2nd December and the new cost will be
Standard DBS check £21.50, Enhanced DBS check £49.50 + any admin costs.
