Job overview

Do you have experience of working with Older Adults with Mental Health issues related to cognitive impairment and Dementia ?

A rare opportunity has become available to join the Dementia Rapid Response Team covering Chesterfield and North East Derbyshire as a specialist support worker. 

The Dementia Rapid Response team support people to live in their own homes and to live well with dementia.

Main duties of the job

What are the hours?

The service runs 365 days a year, Monday-Friday, 8am-8pm, weekends and bank holidays 9am-5pm. This post offers 30 hours / week.

Where would I be working?

Base: Deepdale Business Park, Endcliffe Mount, Ashford Road, Bakewell, Derbyshire, DE45 1GT

Area Covered: The High Peak and North Dales

This job involves traveling to different sites/places in the community. Public transport might not be fast enough for these visits.  

The role:

  • To support service users in their own homes and / or in other settings as agreed, and to undertake tasks in support of service user's care plans.
  • To encourage service users to participate in relevant activities through the development of therapeutic relationships. 
  • To support and assist in practical activities outlined in the care plan
  • To maintain effective record keeping

 NVQ 3 Level NVQ in Health & Social Care or equivalent qualification
