An exciting opportunity for a Band 5 Community Mental Health Nurse has become available in the North Dale and High Peak Community Mental Health Team.

We are an experienced and professional team, dedicated to the goal of providing excellent care for the Older Adult population.

As a team, we are a friendly and supportive; ensuring that we operate with professionalism and inclusivity.

Due to the broad geographical area this post covers the post holder will need to demonstrate that they can travel within the area frequently, transporting Service Users as needed occasionally, without compromising service needs.

 

Main duties of the job

If successful, you will have the opportunity to:

  • Work with Service Users with both functional and organic illnesses, with the full support of the MDT. 
  • Carry a caseload of patients with varying needs and this will include depot injections. 
  • Use excellent assessment skills, risk management skills and communication skills; working towards building therapeutic relationships with both Service Users and their families/carers.
  • Work as part of a knowledgeable, supportive and forward-thinking team who are committed to supporting and guiding learning and development.
  • Work closely with our Voluntary partners, Primary Care and Our Care Homes project.
  • Receive regular Clinical, Managerial and Peer supervision and access a wide range of development opportunities.
  • We welcome applications from newly-qualified  Nurses and offer an excellent Preceptorship programme.
  • Have the peace of mind of knowing the Trust operates a fair and equitable mileage expenses scheme.

Attainment/ Qualifications

Essential criteria
  • Registered Nurse
  • Degree/ Diploma in Nursing or equivalent experience
Desirable criteria
  • willing to undertake assessor qualification

Knowledge/ Experience

Essential criteria
  • Demonstrable knowledge of legislation relevant to Mental Health
  • Working knowledge of the principles of CPA (Care Programme Approach) where appropriate
  • Evidence of Safety Planning
  • Commitment to recovery and person-centered practice
  • Able to Drive and have ability to transport self around geographical area
  • Understanding of Older Adult Patient complexities and Illness
  • Ability to Lone Work
  • Ability to work as a team
  • Good communication skills - verbal/non verbal
