Job overview

We have an exciting opportunity for an Integrated Community Manager to join our team, leading both core and rapid community nursing services across Amber Valley. 

The role will include leading and managing the Community Nursing teams within the Amber Valley Place footprint. The nursing teams are made up of Rapid and Core services. The post holder will also manage the Care Coordinators and Enhanced Clinical Practitioners within the area. The post will work closely within Team Up and the development of integration with system partners. 

These are busy teams which operate to a very high standard.

 

Main duties of the job

As well as providing inspiring and supportive day to day operational management, there will also be lots of scope to participate in service development and transformational change. 

The ICM role will be pivotal in working with the Primary Care Networks facilitating the Ageing Well agenda.

The post is open to qualified Nurses, Occupational Therapists or Physiotherapists.   

The post holder will work very closely with other ICMs within Amber Valley and Erewash, as well as county -wide, with peers who meet regularly for support and project development work. 

Mentoring and leadership development will be supported. 

You will be highly motivated, enthusiastic, have excellent communication and leadership skills and have a current working knowledge of the integrated care agenda.

Your role will be working within DCHS to ensure our staff are engaged and motivated by the changes and that you work in close liaison with our commissioning, social care, primary, secondary care colleagues and other partners to redesign pathways of care.

Experience

Essential criteria
  • Demonstrable experience at an operational management level, ideally within the healthcare industry
  • Proven ability to operationally manage multiple projects at any one time
  • Change Champion – able to demonstrate evidence of delivering a change programme to achieve targets set - experience of effectively applying change management skills in an environment influenced both by internal and external changes
  • Good financial awareness and control
Desirable criteria
  • Experience of key stakeholder management
  • Management and training/coaching experience

Qualification

Essential criteria
  • Current registration as registered nurse or AHP
  • Post graduate management qualification or equivalent experience.
  • Degree qualification or equivalent level of study/work experience.
Desirable criteria
  • Post graduate management qualification (e.g. Diploma in Management Studies) or equivalent experience.
