The post holder will provide care across the two member practices – Royal Primary Care East and Royal Primary Care West.

Key Responsibilities
The post holder's role will primarily involve patient-facing clinical work, supporting the PCN GP practices alongside a focus on PCN initiatives including, innovative at-scale work and opportunities. Additionally, they will offer support to the care homes within the PCN and have the ability to work within the GP Enhanced Access Hub, providing enhanced services to the registered population.
This job description acts as a guideline to the responsibilities of the post holder and will be reviewed and amended to meet changing professional and service needs.

- North Derbyshire PCN has a combined list size of Circa 33,000 patients across the Chesterfield area.
- We have a full ARRS team including Pharmacy, First Contact Physio's, Care Coordinators, Social Prescribing Link workers and GP Assistants.
- The member practices operate in sectors and use TPP SystmOne in a shared admin configuration to allow maximum operational flexibility.
- The member practices enjoy working as a mixed clinical team with GPs, ANPs, Specialist Mental Health Practitioners and Pharmacists providing care to our patients

Qualifications and Training
Essential criteria
- • GP accreditation – CCT registered.
- • Currently on a PCT performers list and not suspended from that list or from the medical register
- • Current GMC registration and license to practise
- • MRCGP examination or equivalent experience
- • In regular clinical practise
Desirable criteria
- • Evidence of further postgraduate educational activities in relevant fields
- • Evidence of CPD activities
Experience

- • Experience of working in general practice in the UK
- • Commitment to and experience of working as part of a multi-disciplinary and skill mixed team environment
