As a leading community provider we are at the forefront of this exciting future, joining-up services in local communities to better meet the individual needs of our patients.

Within Chesterfield we have a Rapid Response Therapist role available to cover Maternity Leave. 

The role requires day-to-day delivery of patient care in our Integrated Community Teams, alongside other Therapists,  Generic Therapy Support Workers, Nurses, Support Workers and Social Care. The role also supports triage of therapy and Local Navigation Hub Referrals. 

Our Rapid Response team work together to provide a service which operates a shift pattern between  8.00am and 18.30pm,  covering 7 days, including Bank Holidays.

The successful candidates will have high levels of personal and clinical credibility and the ability to instantly form effective working relationships in a range of work settings.

You will have the opportunity to really make a difference in ensuring that quality services are delivered both now and into the future based around our patient needs.

We will also work with you regarding your on-going development and revalidation.

The way the NHS works is changing; care outside of traditional hospital settings is quickly becoming a much larger part of what the NHS does.

The Rapid response therapy team provides a same day, next day response to patient needs and supports hospital discharges within the community.

The aims of the service are to avoid hospital admissions were appropriate, support early hospital discharges and provide a short term therapy (up to 2 weeks) assessment and intervention to patients within their own homes. 

Our service provides triage for community therapy services and is also part of the Falls Recovery service within our locality.
