We are looking to recruit NMC registered nurses to work within the Rapid Response Nursing team in Derby City.
Service working hours 8-1830 daily 7 days a week.

Main duties of the job
Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
We are based from Florence Nightingale Community Hospital, their is an expectation to travel across the locality as and when required to meet the needs of our patient group.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient’s privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.
Implement / refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.
