We have an exciting opportunity for a highly motivated registered health care professional to work as an Enhanced Care Practitioner Team Leader.
Main duties of the job
This position entails leading the Community Enhanced Care Practitioners within the Derby City South Primary Care Network. The role involves conducting weekly ward rounds in care homes, ensuring high-quality care and support for residents.
This role will involve working closely with and being responsible to the PCN leadership team.
As well as the day to day management of the team the post holder will have a focus on achieving high quality care and improving services.
There will also be opportunities to work with the wider Management Team to develop collaborative working with stakeholders from health and social care.
As a leader in our organisation it will be your responsibility to ensure that your behaviour and values reflect those required to lead in the DCHS Way.
You will need to engage effectively with your team, peer group and external/internal stakeholders to support you in delivering your key performance measures. This will require an inclusive, transparent, open approach to your role, where you are happy to be accountable to others and promote the concept of a leader who is challenging, whilst expecting to be challenged.

Education, Qualifications and Training
Essential criteria
- BSc (Hons) degree or equivalent clinical qualification
- Nursing or HCPC Professional body registration
- Varied Clinical NHS Experience
- Advance MSC Degree (Any Relevant Studies)
Desirable criteria
- Leadership/management training
- IT qualification
Experience and Knowledge Required
Essential criteria
- Varied clinical experience and multidisciplinary team – multi agency working/management
- Operational management experience (ward or team level)
- Successful proven change management experience
- Experience of dealing with HR issues and conflict management
- Clinical Experience working with frail/elderly patients in care homes
Desirable criteria
- Experience of case and caseload management
Skills and Attributes
Essential criteria
- Good IT skills (microsoft)
Desirable criteria
- Developed coaching/motivational skills
