Job overview
· Provide quality evidence based public health nursing service (health visiting) to a defined corporate caseload to support and enable families and children to make informed choices about their health care and promote health and wellbeing.
Main duties of the job
Provide quality evidence based public health nursing service (health visiting) to a defined corporate caseload to support and enable families and children to make informed choices about their health care and promote health and wellbeing.
Develop knowledge, evidence skills and knowledge to lead on the delivery of the Healthy Child Programme as defined within the service specification. To assess, plan, implement and evaluate the family’s health needs and delivery of care using the Universal, Universal Plus, Universal Partnership plus model as per the Healthy Child Programme and service specification.

The post holder will be responsible for the delivery of health visiting services to a defined corporate caseload within Derby City. The post holder will also be expected to work within the 0-19 framework ensuring a seamless service to the family as appropriate.
