Job overview

At the University Hospitals of Derby and Burton (UHDB) we are looking to recruit an enthusiastic Advanced Clinical Practitioner to join our well-established Outpatient Parenteral Antimicrobial Therapy (OPAT) team. This exciting new role will contribute to the invaluable work provided by the team, ensuring patients requiring intravenous and high-risk oral antimicrobials are discharged and managed safely in the community.

The multidisciplinary OPAT team is led by an Infection Diseases Consultant and you will work closely with the Specialist OPAT Pharmacists and Nurses and broader clinical teams to deliver a safe and effective, patient-centred OPAT service. The OPAT service is well-regarded in the Trust, and this role is pivotal within the team.

You will also have the opportunity be involved in the trust antimicrobial stewardship program to help reduce the rise in antimicrobial resistance. You will support the team in developing a trust wide culture that reduces hospital acquired infection and supports prudent use of antimicrobials.

Advanced clinical practice is well established at UHDB and is seen as integral to the delivery of high quality and safe patient care across the Trust. We have a well-established leadership structure to support ACPs and lots of opportunity for service development and quality improvement work. If successful, you would be joining an extremely progressive team which continues to develop to meet the needs of our patients.

Main duties of the job

We are looking for an ACP who demonstrates that they are highly motivated, patient-focused and exhibit a passion for clinical practice.  A mature and professional attitude is essential and you must be able to establish and maintain good working relationships.

You will be required to demonstrate the above characteristics along with your level of practice through a portfolio of evidence and will be asked to work a period of observed clinical practice.

You will work closely with clinicians, pharmacy and nursing colleagues to support the safe discharge and management of patients requiring OPAT.

The main duties involve assessment and management of patients awaiting discharge and those already out in the community receiving OPAT.

You will also be involved in the monitoring and follow-up of patients, liaison with referring clinical teams and maintaining accurate patient records. Furthermore, you will have opportunities to attend a variety of MDT meetings to contribute to the management of patients.

You will be expected to deputise for the OPAT Consultant at times. There will be lots of opportunity for professional development and involvement in audit/quality improvement work.

Although primarily based at the Royal Derby Hospital, you will also be expected to rotate between the other sites at UHDB as needed.
