Job overview

We are looking to recruit new Community Therapy Support Worker to join our friendly in-patient (Pathway 2) team within the Amber Valley and Erewash area.

The Pathway 2  therapy team  provides rehabilitation and discharge planning support to patients within our local pathway 2 beds. 

Our Amber Valley and Erewash Integrated Community Services are a multi-disciplinary team, providing person-centred holistic care to patients with a wide variety of health conditions.

Main duties of the job

As a Rehab support worker you will be required to follow treatment plans within the patient record and under the direction of the qualified Therapists, and deliver therapy interventions to patients within the bedded care units.

You will be required to do an array of mandatory training related to your role and will be provided with a supervisor to provide 1 to 1 support, to offering you ongoing wellbeing support and assisting you with signing off competencies and closing any skills gaps you may have.

Education, Qualifications and Training

Essential criteria
  • NVQ Level III in care / BTEC Health & Social Care or equivalent
  • Experience of working in adult health care setting
Desirable criteria
  • Additional training in:- Rehabilitation, Palliative Care, Care of the Older person

Experience and Knowledge Required

Essential criteria
  • Previous experience of team working in a health/social care setting
  • Understanding of factors that influence health and wellbeing
  • Understanding of the implications for, and responsibilities of the post for Risk Management and Safeguarding

Other

Essential criteria
  • Able to work flexibly to meet service demands
  • Able to work across daytime shifts and work weekends and bank holidays

Skills

Essential criteria
  • Basic IT skills
  • Good verbal communication skills
  • Able to keep accurate records
  • Well-developed inter-personal skills
