Employed by DCHS NHS FT you will deliver care for the patients within Derby City South PCN.  You will work closely with the GP’s and other staff within the PCN, the HVS Team and the wider DCHS Community Teams. 

This is an exciting time to join the team here in Derby City South as we bring together the Health, Social Care and Voluntary sectors to provide the best care in the right place at the right time for our patients.

Main duties of the job

The key responsibility will be triaging and managing referrals to the HVS and allocating them to the most appropriate clinician in the team. 

You will also provide support, advice & guidance to the team.

Provide day-to-day clinical leadership for the multi-disciplinary and multi-agency service to deliver effective, efficient and high-quality Primary Care services to Housebound Patients.

Mentorship, guidance and support to clinical decision making throughout the whole extended team.

Medical assessment and management on behalf of and in conjunction with members of the team.

Senior clinical triage for the Acute Home Visiting service, to determine the urgency and type of response needed, according to clinical need.

Provide education and mentorship to members of the team, especially those in advanced or extended practice roles, including trainees.

Contribute to quality improvement and learning activities, including audit, service improvement, PDSA cycles and learning event analysis.

Build relationships across the extended team including, but not limited to social care, ambulance, social prescribers, community, and mental health clinicians.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

You will work as part of a small team of Community General Practitioners to support a multi-disciplinary team for Derby City South PCN.

This is direct work with patients and their carers either in person or remotely. 

You will also contribute to the development of Team Up & the HVS working with other clinicians.

Working in conjunction with the PCN Leadership & Management Team, Home Visiting Service clinical lead(s) and Community GP(s).

You will contribute to developing and delivering a safe and effective service, developing new ways of working and clinical pathways in accordance with key local and national clinical standards, for the service areas of Acute Home visiting, urgent community response, enhanced health in care homes and anticipatory care.

You will work with key stakeholders and partners locally to contribute to the ongoing development of the service, promoting a cross organisational and multi-agency approach to the delivery of care for local residents.

Person specification

Education, Qualifications and Training

Essential criteria
  • Doctor/Consultant on the relevant Specialty Register
  • Current registration with GMC
Desirable criteria
  • Royal College membership
  • Mentorship and Supervision Qualification or equivalent experience (e.g. GP Trainer, DMP)
  • Qualification or equivalent experience in delivering teaching programmes

Experience and knowledge

Essential criteria
  • A good knowledge of and interest in Community care
  • Experience of working in multidisciplinary teams
  • Experience of working with patients living with frailty, or a willingness to develop skills in this area with Trust support (If that is your desired area of alignment)
  • Experience of medical education/clinical supervision
  • Ability to work effectively and provide leadership across traditional organisational and professional boundaries
