Job overview

An exciting new opportunity has arisen for a Band 6 Mental Health Practitioner with the North Hardwick & Bolsover PCN in Derbyshire.

You will be a valued and respected colleague within both the PCN and DHCFT team. This role is an exciting opportunity to work closely with colleagues in the GP surgeries.

You will be able to help educate and support teams within primary care, whilst supporting and delivering short term interventions to patients with low to moderate mental  health problems.

Main duties of the job

The service provides appointments for North Hardwick & Bolsover General practice registered patients. The role will provide opportunity to work within the Primary Care 

You will have opportunity to work with the pcn and gp practices to help shape the triaging system. You will consult with patients within the North Hardwick & Bolsover Practices; this will be a combination of remote consultations and face to face depending on need.

Your role will be a combination of supporting with triage and managing your own intervention consultations .

The majority of the Mental Health Practitioner role consultations are to support patients with low to moderate mental health problems in the GP practices 

You will consult, taking a brief focused history and conducting a risk assessment, formulation of need and onward plan.

You will work both autonomously and as part of a wider team.

You will also have opportunity to support general practice staff within the North Hardwick & Bolsover primary care network.  This support will build the confidence of staff in managing mental health, will build the resilience of individual members of staff and improve quality.

  • To provide primary care based first contact appointments inclusive of advice, enhanced assessment, support and initial treatment (where appropriate) for those experiencing problems with their mental health and emotional wellbeing.
  • To provide clinical advice and support to non-mental health clinicians in a PCN.
  • To work across the interface between primary care and Locality Team specialist mental health services.
  • Responsibility for the delivery of high quality patient care within the service area.
  • To work as an accountable and autonomous practitioner able to recognise changing phase of illness and health and social care needs of patients with serious mental illness where such patients present and to refer appropriately.
  • to practice autonomously in making clinical judgements, identifying alternative courses of action, managing and providing a high-quality care.
  • To be a key member pf the PCN multi-disciplinary team, attend and engage with the PCN MDT. Where applicable actively contribute to wider system MDTs as they develop.
  • To undertake initial formulation of need by telephone/video. Record consultations in the SystmOne General practice records.

Qualifications

Essential criteria
  • Nursing Degree or equivalent qualification ins mental health social work / OT

Experience

Essential criteria
  • experience in working with patients with severe and enduring mental health
  • Experience working in a community setting
  • Experience and knowledge in psychological interventions
  • Experience and knowledge in assessments and care planning

Skills

Essential criteria
  • Excellent communication skills - verbal and written
  • Good use of computer skills
  • Experience of working within a Multi Disciplinary Team
Desirable criteria
  • Driving licence with access to a car for work purposes
