Main duties of the job
To manage referrals into our service and provide evidence-based care to support patients in their own home.
This great opportunity includes working alongside general practice, therapy and our Home visiting colleagues.
To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.
To provide advice and support to junior staff in the delivery of high quality care and actively support the community nursing sister/charge nurse in the management of the health care environment.
