Senior Community Children's Nurse

We are looking for experienced Children’s Nurses RSCN or RN (child) to join our Children’s Community Nurse Pathway. We could be flexible about where this 22.5hr post is based across our Devon nursing team. This position includes provision of support out of hours and the postholder will need to support this work on call on a rota basis.

You should have proven experience of working with children who have complex disability and or life limiting/ threatening conditions, who can demonstrate the ability to work well within a team environment, be able to work autonomously and ideally have community nursing experience.

You will care for children with a wide variety of health conditions providing nursing care to promote care close to home and avoid hospital admission. You will work in partnership with other health service providers and other agencies, both statutory and voluntary, to provide exceptional levels of nursing care to our patients and strive to continually improve care with the involvement of patients and families.

  •  To provide a robust training programme to families and carer’s in addition to the wider CCN team in order to promote safe care delivery in a variety of community settings.
  • To provide specialist nursing support, advice and interventions within the multiagency ethos of children and family health services for children and their families. 
  • To assist in promoting the health and well-being of children with complex nursing needs, to enable them to achieve the maximum benefit from their educational opportunities.
  • To provide case management of an identified caseload of children with highly complex and/or long term conditions by utilising and promoting evidence based care.
  • To provide nursing care, support, training, advice and education to children, families and carers in order to meet the children’s health needs in the school and community environment.
  • To provide children and young people with the opportunity to express their views in an individually appropriate way, having to have their opinions taken into account to develop the services
  • To support symptom management at end of life within the community setting including providing on call cover for provision of care outside of normal working hours

The Children’s Community Nursing pathway works across Devon to provide nursing care and is currently comprised of three teams; the Children’s Community Nurse team, Special School Nurse team, and the Palliative Care team. The teams work closely together to provide a high quality responsive and dynamic service for children and young people in the community setting across Devon.

  • Current registration with NMC

  • Community Specialist Practitioner Degree or diploma (or substantial documented experience of working in the community).
  • Degree level qualification in nursing.
  • Non-medical prescriber V100 or V300

Experience

  • Significant experience of working within the community / primary care setting.
  • Proven experience of working with children with complex medical needs, special needs and life limiting conditions.
  • Able to work collaboratively within a multi-disciplinary interagency team.
  • Ability to prioritise own workload, balancing numerous demands and meeting deadlines.
  • Evidence of the implementation of risk and assessments.
Meini prawf dymunol
  • Experience of Children & Young People with tracheostomy and long term ventilation
  • Training qualification or experience

Knowledge

  • Knowledge of NHS IT systems
  • Ability to maintain accurate records within professional and organisational guidelines and process.
  • Good working knowledge of Children's safeguarding processes
