Job overview

We’re launching a new Healthy Ageing (Frailty) Hub at Newton Abbot Community Hospital to deliver rapid, person-centred care closer to home. As a Locally Employed Doctor (LED) (CT1/2 equivalent), you’ll be based in our Frailty Same Day Emergency Care (SDEC)/ambulatory service, working within a supportive multidisciplinary team to assess, investigate and manage adults living with frailty. No routine out-of-hours—some Bank Holiday cover may be needed to maintain safe medical support. We are recruiting two posts and will discuss hours on appointment.

Main duties of the job

You will be expected to abide by the code set out in the GMC “Good Medical Practice” and to continue to improve on the generic competencies outlined in the Foundation Programme.

Other responsibilities include:

  • Clerk SDEC patients and commence elements of Comprehensive Geriatric Assessment (CGA) (training provided)
  • Participate in daily MDT huddles, liaising with nursing, therapy, ACP/GP and senior medical staff to plan safe, timely care.
  • Complete discharge summaries and contribute to smooth handovers across virtual ward/UCR/community services.

Detailed job description and main responsibilities

  • You’ll work within the new Community Healthy Ageing Hub model, where patients are triaged via a frailty coordination function and streamed to the most appropriate pathway—SDEC/ambulatory at Newton Abbot, virtual ward at home/care homes, or community/intermediate care—supported by UCR and wrap-around services.
  • Your clinical responsibilities will include clerking and early CGA for SDEC patients; safe clinical decision-making with senior support; documentation and discharge/transfer summaries; proactive liaison with virtual ward/UCR/community teams to avoid unnecessary acute admission where safe.
  • Work within Trust policies and contribute to education (students/teaching). The service is brand new and will rapidly develop, so you’ll bring a flexible, compassionate approach and help us refine pathways that improve outcomes for people living with frailty
