The Harbour Frailty Same Day Emergency Care (SDEC) service at Newton Abbot Hospital is launching, and we are seeking an experienced Sister / Charge Nurse to support the coordination and day-to-day running of this innovative new unit.

The Harbour provides a patient-centred alternative to hospital admission, delivering same-day assessment, treatment and discharge for older people living with frailty. This role is key to ensuring a smooth patient journey, maintaining high standards of care, and supporting staff within a fast-paced multidisciplinary environment.

As the Band 6 Sister / Charge Nurse, you will:

  • Take charge of the coordination of the unit and the shift
  • Provide clinical leadership and act as a role model for the team
  • Support safe, effective patient flow and high-quality care delivery
  • Work closely with the MDT, patients and families
  • Support staff development and maintain Trust and NMC standards

Main duties of the job

Working for our organisation

We are looking for a passionate and committed Band 6 ward sister/ charge nurse who has an interest in Frailty /Elderly care/ dementia care or is interested in gaining a broad knowledge around this fantastic area of nursing.

You will work as a Registered Nurse within a multidisciplinary team. To promote and maintain a safe clinical environment for all patients, their carers, visitors and staff.

To comply with the Trust's values and expected behaviours and deliver a high standard of care in collaboration with the multidisciplinary team.  

To ensure care, assessments, treatment and documentation is delivered in line with Guidelines and to act as an advocate for patients, their families and staff.

Detailed job description and main responsibilities

  • To provide a high quality service which complies with relevant legislation and NHS Employment check standards.
  • To be honest and learn from mistakes, and help to create a “no-blame” culture were people feel able to share and learn from experiences together.
  • Support and develop junior staff, act as a role model and lead the way to excellent care.
  • Lead Board/MDT rounds (SAFER),  delegate explicit roles and responsibilities.
  • Keep up to date with Organisational developments, service delivery, clinical and administratively.
  • Adhere to the NMC code,  Trust protocols, procedures, Job description and Contract. 
  • Have an awareness of Budget and expenditure, not wasteful of resources.
  • Be competent at using and supervising a wide range of specialised Equipment and  confident with training.
  • Support Staff in reflection, and critical thinking/learning.
  • Work alongside the ward manager to develop your own knowledge and skills.
  • Follow NMC Documentation guidance, alongside TSDFT Policies, and stay up to date with the Trusts Innovative IT systems.

Essential criteria
  • Current NMC registration
Desirable criteria
  • Post graduate diploma degree in relevant subject or equivalent level experience

Knowledge and Experience

Essential criteria
  • Recent evidence of meeting PREP requirements
  • Able to assess, plan, implement and evaluate patient care
  • Knowledge of clinical governance
  • Knowledge of individual responsibility to health and safety and clinical risk
Desirable criteria
  • Awareness of the NHS plan and current initiatives
  • Knowledge of ICO initiatives within Torbay Trust
  • Negotiation skills
  • Diverse and adaptable communication skills to enable effective communication with the MDT
