Care to join us as a Permanent Band 3 Intermediate Care Support Worker?
You will assist Occupational Therapists, Physiotherapists and Community Nurses working under supervision to provide rehabilitation to patients within a rehabilitation unit to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge and maximise independent living within the recommended time frame of six to eight weeks.
Main duties of the job
As a Support Worker, you will take part in the Baywide Intermediate Care weekend rota and late duty rota across Torbay and the Jack Sears Unit.

You will make a difference by helping patients through recovery and returning back to health and independence.
There might be a requirement to support community rehabilitation, intermediate care and urgent care dependant on needs of the service.

Detailed job description and main responsibilities
- Communicate with a range of people on a range of matters that is appropriate to them and the situation
- Interact and communicate with patients, carers and relatives appropriately and effectively taking behaviour and body language into account in order to ensure information is understood by all concerned
- Communicate effectively with those who may have a physical and or mental impairment, e.g. loss of sight/hearing, depression/anxiety and stroke
- Skilled in sensitively ensuring patients are aware of their limitations and that all goals are, specific, measurable, achievable, realistic and time bound
- Participate in the review or case conference of individual clients and contribute to the evaluation and development of their progress in reaching goals

Qualifications and training
Essential criteria
- NVQ 3 in Health and Social care
- Good written and verbal skills
- Driving licence and access to a vehicle
- Basic IT skills
Desirable criteria
- Experience of rehabilitation in a number of settings
Knowledge and experience
Essential criteria
- Experience of working within multidisciplinary team/multiagency team (in hours)
- Ability to communicate effectively with diverse groups of people
- Ability to keep accurate reports according to organisational procedure
- Ability to work in a way that minimises risks to health safety and security
- Understands need to work within the limits of own competence and responsibility and refer issues beyond these to relevant people
- Ability to recognise and report behaviour that undermines inequality and diversity
- Ability to work in a way that is in accordance with legislation, policies, procedures and good practice
- Evidence of continuing professional development
Desirable criteria
- Familiarity with IHCS and PARIS computer systems
