Job overview

University Hospitals Plymouth NHS Trust is the largest hospital in the Southwest peninsula and the major trauma centre (MTC) for the South West of England within the Peninsula Trauma Network, providing comprehensive secondary and tertiary healthcare.

The Emergency Department (ED) currently sees more than 100,000 patients per year. The Peninsula is a largely rural region with very few centres with a large urban population. The overall population of the region is approximately 1.67million, which can increase by an extra 1 million people in the peak holiday season.

The ED facilities currently include a 7-bay resuscitation room (with co-located CT scanner), 21 majors cubicles, a separate ambulatory majors area, a minors area, a dedicated paediatric area, 2 small theatres and a plaster room, a seminar room, break room, and offices. A £160m rebuild of the ED is planned to commence in early 2023, with a scheduled opening in November 2025, as part of the Future Hospitals Programme. This will provide a state-of-the-art ED suitable for our growing population.

Main duties of the job

There is an establishment of 28 consultants (approximately 20 WTE), 30 Middle grade doctors (including one Associate Specialist, nine Higher Specialty Trainees and four CT3 level doctors), with additional junior doctors, Acute Care Practitioners and Nurse Practitioners sharing the workload. There is an establishment of nursing staff, led by a matron, and supported by a variable number of Ministry of Defence nurses and MAs.

Our department also benefits from close links with the military, the British Antarctic Survey Medical Unit (BASMU), Devon Air Ambulance (DAAT), and the medical school. We have a strong academic department undertaking our own research and portfolio studies. We are a teaching hospital with significant input into undergraduate medical, nursing and paramedic education, along with postgraduate training at all levels.

Detailed job description and main responsibilities

The post holder will be expected to undertake full clinical duties including the immediate care of critically ill patients attending the Emergency Department, and patients presenting with injuries both major and minor. The care will include initial resuscitation where appropriate, diagnosis, management and the treatment, referral and discharge under the guidance of the Consultant. The referral could be to GPs, to an appropriate specialist team for continued hospital care, or to other services.

You will be expected to follow the various guidelines and protocols issued in the department. In the case of major trauma cases, cardiac arrests and critically ill patients attending the department, the post holder will be expected to act in a team leadership role, following appropriate life support guidelines. Post holders would be expected to hold certification in ALS, ATLS and gain APLS (or recognised equivalents to these courses) and maintain these.

The area of Specialist interest will be discussed at interview, and an agreement will be made to include the time allocated to this area as well as some structured and achievable objectives. This may take the form of a project, further qualification etc. 

The rota is designed to be as flexible as possible and is both annualised and self rostered.  A full time work schedule is 40.3 hours. Weekends will be 1 in 3 and there are a total of 9 nights and 9 lates in a 12 week rota cycle. The fellow time will be in lieu of midweek day shifts.

Study leave time will be available for relevant educational activities – the entitlement will be at the discretion of the clinical / rota manager.

 

Person specification

Education, qualifications & special training

Essential criteria
  • MBBS or equivalent medical qualification Holds GMC Licence to Practice at time of application. ATLS/ETC and ALS provider status (or equivalent)
Desirable criteria
  • Postgraduate qualifications in Emergency Medicine or other evidence of academic and professional achievement. APLS/EPALS provider. APLS / ATLS / ETC/ ALS instructor. Relevant clinical and teaching courses. MRCEM exams undertaken.

Particular skills and experience

Essential criteria
  • At least 2 years’ experience following on from foundation training, of which at least 1 year in emergency medicine OR completed ACCS training equivalent. (ST3 and above equivalent) Up to date clinical knowledge. Everyday ED clinical procedural skills. Evidence of career progression. Ability to work as part of a team.
Desirable criteria
  • Previous experience as ED middle grade. Leadership skills. Good time management. Desire to pursue a career in Emergency Medicine. Previous UK/NHS experience.

Clinical Research / audit

Essential criteria
  • Willing to take part in clinical audit and research.
Desirable criteria
  • Evidence of completed audit projects. Record of active participation in research. Presentations or publications.

Teaching and learning

Essential criteria
  • A genuine interest in clinical education. Ability to teach clinical and practical skills. Recognition of uncertainty and one’s own limitations.

  • Evidence of teaching experience. Postgraduate teaching qualifications. Evidence of experience in mentorship.
