Hepatology Advanced Clinical Practitioner

Job overview

-         The Lead Hepatology ACP & team leader will create, lead, and be responsible for maintaining a nursing culture which is open to change, new ideas, concepts and innovation.

-         They will provide inspirational and motivational clinical leadership to their team of nurses to ensure the highest standards of nursing care.

-         They are a role model for all staff and will excel at collaboration with the multi-disciplinary team.

Fulfilling the expectations and core capabilities of an ACP as set out in the Trust ACP Job Description.

Main duties of the job

The post holder will be able to:

-         Work as a high level practitioner with advanced clinical skills within an interdepartmental and / or multidisciplinary team primarily to ensure that patients within their sphere of clinical expertise receive holistic, timely and high quality clinical care.

-         Exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care, whilst maintaining a professional portfolio that supports their scope of practice.

-         Working autonomously in conducting clinical examination, formulating a working diagnosis and providing timely treatment interventions in accordance with their scope of practice 

-         Monitor and lead improvements to standards of care through; supervision of practice, clinical audit, implementation of evidence based practice, teaching and support of colleagues and the provision of professional leadership.

-         Lead the implementation of new evidence based practice and contribute to the development of the evidence through research, audit and collaboration with clinical or academic staff.

-         Contribute to clinical governance by leading quality improvement, audit and research projects as part of the multidisciplinary service-wide performance and quality agenda.

Works in accordance with the Multi-professional framework for advanced clinical practice in England (HEE, 2017) the University Hospitals Plymouth (UHP) Trust Framework for Advanced Practice.

Detailed job description and main responsibilities

Clinical Practice

1.     Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for patients by virtue of expert clinical judgement and close liaison with members of the MDT. 

2.     Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

3.     Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, life style advice and care.

4.     Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families and carers.

5.     Ensure compliance with their respective code of professional conduct and work within their scope of practice.

6.     Works in conjunction with the MDT to clearly define values, direction and policies impacting upon care.

7.     Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate

8.     Be able to demonstrate a critical understanding of their broadened level of responsibility and autonomy. This includes being critically aware of their limits of their own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.

 

10.  Utilise professional judgement and maturity, understanding when to seek help.

11.  Demonstrate critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.

12.  Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make positive changes, using Health Education England’s framework to promote person-centred approaches in health and care

13.  Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining and evaluating links to manage risk and issues across organisations and settings.

14.  Evidence the underpinning subject-specific competencies i.e. knowledge, skills and behaviours relevant to the role setting and scope, and demonstrate application of the capabilities to these, in an approach that is appropriate to the individual role, setting and scope.

15.  Act as a clinical role model/advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies and networks.

16.  Actively implement effective systems and processes for infection control management whilst working clinically and be a role model for other members of the clinical team.  

17.  Initiate drug therapy/medication within the parameters of agreed clinical guidelines and in accordance with the legal framework you work under to supply and prescribe medicines (if from professional group legally entitled to prescribe)

1.     The post holder will provide speciality high risk procedures examples are – Therapeutic and diagnostic Paracentesis with the use of bedside ultrasound/ ultrasound guided cannulation / peripherally inserted central catheters / midlines and difficult phlebotomy.

2.     The post holder reduce the number of new outpatient appointments / inpatient admissions to hospital by streamlining our current patient pathways and creating new ones.  

3.     The post holder will recognise early signs and symptoms of liver disease exacerbation as well as when patients are entering the terminal phase of their disease trajectory.

4.     The post holder will facilitate the patient’s preferred place of care by engaging with the advanced care planning MDT and End of Life register.

5.     Use advanced expertise in health promotion and the prevention of illness to assess and advise patients with reference to alcohol, smoking cessation, diet, recreational / illicit drug use, mental health, immunisation, sexual health

6.     Use highly developed knowledge and skills to assist and support the patient in developing their understanding and skills to ensure effective self-management of liver disease as a chronic disease process

 

Leadership

7.     Role model the trust values, demonstrating a person-centred approach to service delivery and development.

8.     Lead new practice and service redesign solutions in response to feedback, evaluation and need, working across boundaries and broadening sphere of influence.

9.     Critically apply advanced clinical expertise to influence clinical practice to enhance quality, reduce unwarranted variation and promote the sharing and adoption of best practice.

10.  Evaluate their own practice, and participate in multi-disciplinary service and team evaluation, demonstrating the impact of advanced clinical practice on service function and effectiveness, and quality

11.  Continually develop practice in response to changing population health need, engaging in horizon scanning for future developments

12.  Pro-actively initiate and develop effective relationships, fostering clarity of roles within teams, to encourage productive working.

13.  Actively engage in peer review to inform own and other’s practice, formulating and implementing strategies to act on learning and make improvements.

18.  Makes representation as appropriate at various meetings, providing feedback to the organisation on clinical and professional issues which have an impact on care and standards of practice within their sphere of responsibility.

19.  Lead in business planning and the development of financial bids for service development.

20.  Discuss patient and relatives' experiences with them to prevent complaints from developing and facilitate investigating and responding to complaints when they arise.  

21.  Lead in the investigation of clinical incidents and serious incidents. Leads and facilitates others to share lessons learned adapt systems and processes accordingly.  

22.  The post holder will collaborate with the Hepatology Service Line director and management team to ensure that the department maintains and is compliant with the Improving Quality in Liver Services national accreditation and standards framework.

Management

 

Responsibility for finance and physical resources:

1.     Ensure the hepatology nursing service has sufficient and competent staff to provide effective and high quality patient care.

2.     Authorise overtime for nursing staff to agreed levels.

3.     Ensure adequate staffing levels and skill mix through active management of staff absences, including planned and unplanned absence

4.     To be the named person to authorise payments from the Hepatology budget and provide the service line accountant with relevant information to ensure financial balance is achieved.

 

Responsibility for Human Resources

5.     Take overall accountability for ensuring annual reviews and personal development plans are completed for all permanent nursing staff.

6.     Participate in the recruitment and selection of high-quality staff, ensuring that new staff receive an effective induction.

7.     Manage performance issues, providing support and clear objectives and if appropriate use the relevant policies and procedures.
