The Associate Medical Director (AMD) for Primary Care Liaison will provide Strategic and Clinical Leadership to strengthen collaboration and partnership working between University Hospitals Plymouth NHS Trust (UHP) and local primary care providers, including GP practices, Primary Care Networks (PCNs), and the wider Devon Integrated Care System (ICS).

 

The role will focus on improving collaboration across the primary, community and secondary interface, supporting our collective priorities for pathway redesign/improvement, patient flow, reducing system inefficiencies, and supporting integrated care pathways to deliver high-quality, sustainable healthcare for the population served by UHP. 

Main duties of the job

Strategic Leadership

           Act as the senior clinical link between UHP and primary care, ensuring alignment with Devon ICS priorities and UHP’s strategic objectives.

           Contribute to the development of integrated care models that support elective recovery, urgent care flow, and population health management.

           Contribute to the shared work programme at GP collaborative board

           Identify key friction points in the interface (referrals, discharge, follow-up, communication, diagnostics access, shared care) and lead targeted improvement programmes.

Clinical Engagement & Liaison

           Build strong, collaborative relationships with GPs, PCN Clinical Directors, and Local Medical Committees (LMC). Divisional Directors and Hospital Senior Medical Staff.

           Represent UHP at ICS forums and primary care engagement meetings, ensuring secondary care perspectives are understood and integrated. To act as an internal voice to raise awareness aid understanding of primary care in the acute setting

Detailed job description and main responsibilities

Service Improvement & Integration

           Lead initiatives to streamline referral processes, discharge planning, and electronic transfer of pathology results and clinical documentation.

           Support the development of shared care protocols for long-term conditions and high-volume pathways (e.g., frailty, diabetes, cancer follow-up).

          Support the development of care pathways across the primary, secondary, community interface as prioritised by the collaborative forum.

 

Quality & Governance

          Ensure robust clinical governance across joint initiatives with primary care partners.

          Monitor and report on quality metrics related to the primary-secondary care interface, including patient experience and safety indicators.

Education & Workforce Development

           Promote education and training opportunities for primary care colleagues in collaboration with UHP specialists.

           Support workforce planning initiatives that address capacity and capability across the system.

Innovation & Digital Transformation

           Champion digital solutions that improve interoperability between UHP and primary care systems (e.g., GP Connect, shared care records).

           Work with IT and clinical teams to optimize automation and efficiency in information exchange.

Qualifications

Essential criteria
  • GMC registration and license to practice

Knowledge

Essential criteria
  • Significant experience in clinical leadership within secondary or primary care
Desirable criteria
  • Experience in service redesign and quality improvement projects
  • Familiarity with digital health solutions and interoperability standards
  • Previous involvement in ICS-level strategic planning or transformation programmes

Aptitudes & Abilities

Essential criteria
  • Strong understanding of NHS structures, ICS priorities, and primary care operations
  • Excellent communication, negotiation, and stakeholder engagement skills
