A 12 Months Secondment or Fixed Term Opportunity for a Senior Social Work Practitioner in The Mental Health Adult Social Care Team. 

      Part Time 35 hrs per week,  Band 7 (  plus enhancement of 11.8% if you are qualified AMHP)

 

  • To be a core member of a multi-disciplinary Mental Health team and contribute to the effective provision of mental health services to working age adults in Torbay. 

·                 If you have an AMHP qualification and are on the AMHP, this will attract and enhancement on the salary of 11.8% for which there is expectation that you will commit to the Torbay and South Devon AMHP Rota for a minimum 2 duties per month

·                 To lead on undertaking holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust/Devon Partnership Trust standards, policies and guidelines.

·                 To have clear responsibilities for the safeguarding of adults and children

·                 To be compliant with and practice utilising social care legislation including Care Act 2014 and  Mental Capacity Act 2005

·                 To be able to make a positive difference in people’s lives, by working in partnership with service users, their carers and families  

 

 

Main duties of the job

  • The post holder will work within the Torbay and South Devon NHS Foundation Trust’s Excellence Model of Social Work and to promote its practice and values and to support the development of the highest quality social work
  • To Jointly manager with the Team Lead staff, resources, and relationships with key agencies to ensure timely and effective assessment, personalised support planning, and care management. Work in partnership with all agencies to ensure, a high quality, strength base provision is delivered to services users experiencing mental health problems, there family and carer
  •    To jointly with the Social Work Lead in providing leadership and management to a multi-disciplinary team of practitioners
  •  To be responsible for supporting and guiding practitioner in respect of risk assessment and risk management
  •  Guide and support staff in respect of highly complex cases offering a shared case management approach for such complex cases.

     You will be working in the Adult Mental Health Social Care Team and working jointly with the Social Work Lead, you will be a core member of the management team, providing leadership to the multi-disciplinary U65 Mental Health Social Care team. Leading on effective assessment and discharge planning with working age adults in Torbay and for Torbay residents in out of area hospitals in the region.

 

To provide leadership to the team in maintaining the highest standards in strength based holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for The Devon Partnership trust policies and guidelines.

 

To have clear responsibilities for the safeguarding of adults and children and for operating in a manner consistent with Human Rights legislation

 To ensure the practitioners maintain a high standard of legal literacy and are compliant with and practice utilising social care legislation including Care Act 2014, Capacity Act 2005, Mental Health Act 1983.

  • Planning and Organisation

·       Ensure statutory requirements and guidance; national directives and TSDNFT policy and guidance including procedures and protocols for safeguarding adults and children are met and applied in all areas of team/individual activity

Qualifications and training

Essential criteria
  • Relevant professional Social Work qualification – Degree/Diploma in Social Work, Certificate of Qualification in Social Work or Certificate in Social Services
  • Current registration with SW England
  • Professional knowledge acquired through degree/diploma in social care, supplemented by further specialist knowledge, for example in needs of a specific client group, experience to master’s equivalent level
  • Current registration with the General Social
  • PQ and commitment to post qualification training

Knowledge and experience

Essential criteria
  • Experience in a leadership/management
  • Role modelling good practice for social care staff within the multi-disciplinary team
  • Experience of Court Work i.e. Court of Protection
  • Experience of working with complex mental health clients
  • Experience of providing formal supervision
  • Ability to plan and direct complex service demands
  • Experience of chairing meetings i.e. Safeguarding, Best Interest
