Job overview

We are looking for an individual to lead the diabetes podiatry service at Torbay and South Devon NHS Foundation Trust potentially working across 10 locations. This is an exciting opportunity to work at Advanced Practitioner level.

You will have a proven record of excellence in providing diabetes podiatry and the ability to function as a sole practitioner. Experience of working and leading within a multi-disciplinary team and highly developed clinical skills are essential. The role involves the management of a high-risk caseload in both in-patient and out-patient settings, teaching and undertaking research work. You will work closely with the community podiatry team to provide a seamless service to the population provided by our department.

Staff are supported with appraisal (PDP), dedicated CPD time and in the longer term, rotation onto internal hospital-based training.

Main duties of the job

  • Work under the 4 pillars of an Advanced Practitioner
  • Be responsible for the management of a caseload of high risk patients, providing a comprehensive assessment of need, relevant health education and referral to other health care professionals as required.
  • The main duties of this post are to act as the Advanced Practitioner in the field of Diabetes and wound care for both Primary and Secondary Care as part of a Multi-disciplinary Team providing cover for the team,
  • Carry out home visit for housebound patients

Qualifications

Essential criteria
  • • Degree/ diploma in core profession area
  • • Registered on NMC/ HCPC
  • • Master’s degree in advanced Practice or HEE e-portfolio equivalent
  • willing to undertake the ACP masters as a trainee at Band 7
Desirable criteria
  • • Leadership or management course/qualification
  • • Evidence of active research presence
  • • Experience of implementing service improvement projects
  • • Education/Mentorship/Supervision Course

Knowledge

Essential criteria
  • • Extensive experience within diabetic foot and wound care
  • • Advanced assessment skills
  • • Change management skills
  • • Proven experience in risk management
Desirable criteria
  • • Advanced debridement skills
  • • Implementing policy and clinical guidance
  • • Advanced vascular assessment
  • • Multi-professional supervision
