Job overview

Are you an experienced Registered Nurse looking to join a friendly and motivated Out of Hours (OOH) Community team?

We are a supportive multi-disciplinary team who are passionate about delivering high quality care to housebound patients, in partnership with primary care and secondary care, covering moor-to-sea, coastal and Torbay districts.

Joining our team as Band 5 Registered Nurse, you will provide responsive, planned and unplanned support to patients requiring urgent nursing within their own homes and residential care settings.

We work in small teams of two which includes yourself and one Healthcare Support Worker (HCSW) where you will work in partnership throughout your shift, this will include shared driving. Whilst working for our Out of Hours team you will have access to Trust provided Pool Cars for all of your work-transport needs to ensure you can deliver planned and unplanned care.

 

Main duties of the job

  • Working in partnership with HCSW to support patients and their carers
  • Treating Post non-injury or minor injury falls which can include wound dressings and supporting patients with catheterisation, bowel care and treatment escalation plans.
  • Responsible for Community Nurse Equipment i.e., Scales, Bladder Scanner, Doppler
  • Guide and mentor junior staff and students
  • Escalate any concerns to team leaders or Community Nurse Lead
  • Generate prescriptions and order pressure relieving equipment using available IT systems in place.

  • Promote self-care with health education when visiting patients and their carers/family in order to increase understanding and promote maximum independence.
  • Undertake Specialist Nursing and Continuing Care assessments (with appropriate training)
  • The ability to work independently and as part of a team, managing own clinical caseload on a day to day basis
  • Ensure confidentiality of patients and their care is maintained, demonstrating knowledge of principles and purpose of data protection - Record, retrieve and access information in various formats
  • An ability to provide and receive complex, sensitive and contentious information: using empathy and reassurance skills
  • Can provide motivational and negotiation skills in areas where consent/ co-operation is required or there may be barriers to understanding. To include undertaking best interest decisions
  • Ensure timely and effective communication with the team manager and other team members on professional matters
  • Liaise with patients/relatives who could be unpredictable/or challenging in their responses because of stress related problem

Qualifications and training

Essential criteria
  • Qualified Registered Nurse Level 1
  • Evidence of continuing professional development
Desirable criteria
  • Community Nurse Prescriber V150
  • Diabetes course
  • Palliative Care Course
  • Mentorship Course
  • Tissue Viability/ Leg Ulcer course
  • Experience of continuing health care assessments.
  • Experience of COPD or Heart Failure

Knowledge and experience

Essential criteria
  • Proven relevant experience in a range of clinical skills
  • Relevant I.T. knowledge/experience
  • Awareness of safeguarding adults and children
  • Awareness of mental capacity act.
  • Holistic client assessment using an identified model of care
  • Ability to make sound clinical judgements and decisions
  • To maintain timely and accurate written records
  • To manage/prioritise a caseload, demonstrating appropriate delegation
  • Ability to demonstrate relevant team worker experience
