Discharge Key Worker - Stroke Unit

Band 3

A fantastic opportunity has arisen to work on the stroke unit supporting the discharge pathways for patients admitted to our 36 bed unit. The discharge key worker will support the clinical leader and the MDT in ensuring patients can achieve a timely and safe discharge and helping to identify those patients with more complex discharge needs are robustly and proactively managed.

The post holder will need to be confident, able to use their own initiative with excellent communication skills to interact with patients families and clinical professionals.

Having knowledge of local best practice to manage effective discharges will be an advantage as well as some healthcare knowledge.
