Job overview
We are looking for a motivated Advanced Clinical Practitioner to join our Multi-Disciplinary team of qualified and trainee ACP's working in Older Persons Same Day Emergency Care (SDEC) at UHD. SDEC is a fast paced and dynamic environment that aims to avoid admissions in line with the National Strategy for SDEC and has close working links with our hospital at home team and our Older Persons Admissions unit.
If you are a good communicator with a passion for improving the care of Older People living with Frailty and complex co-morbidities, then we would like to hear from you. You will need to be able to work autonomously as an ACP and have a keen interest in working in Older Persons Services and be a role model to our trainee ACP's and support staff.
The successful post holder will hold an accredited MSc in Advanced Clinical Practice or have completed the e-portfolio route with digital badge and be willing to complete our local competency booklet evidencing proficiency in the care of Older People.

- Demonstrable theoretical and practical knowledge of OPS specialty common presentations
- Participation in research and experience of critical appraisal of the evidence base to inform best practice
