Job overview

An exciting opportunity has arisen to join an existing UHD multidisciplinary team, working on the Poole and Bournemouth sites, but also undertaking outreach to review patients in the community. 

The aim of the service is to care for patients accessing  UHD services who are homeless or at risk of homelessness and who may also have mental health, alcohol and drug misuse issues. The role will also involve the care of patients undergoing treatment for Chronic Hepatitis C across Dorset as part of the Dorset Hepatitis C Elimination Programme. This will involve working with partners in the community and on a mobile clinical van. 

The service also aims to enable patients to receive the best care that can be provided through improving staff education, acute care delivery and pathways to community services.

You will be someone who enjoys the challenges this type of work represents and has a passion for caring for this patient group. Someone who is used to multi-agency, multi-disciplinary working in partnership with many agencies and organizations and is confident to see patients in hospital at the bedside, at the front door, as well as in the community, including seeing patients in their own homes, hostels etc with appropriate support,  risk assessments and safeguards in place. 

This post requires an active UK NMC Registration
