The ACP role is a cutting-edge role in the field of advanced clinical practice which crosses professional boundaries, undertaking traditional medical roles and leading inter-professional practice. It is a role model of excellence in the delivery of holistic care to patients.

The ACP will be able to assess and treat the many varied clinical cases; both acute and chronic. The post holder will provide clinical expertise for developing and enhancing the service, demonstrate advanced skills and competencies and ensure that evidence-based practice is inherent in all aspects of care and treatment. 

The ACP will support service delivery and leadership working alongside colleagues in support of the education and development of both nursing, midwifery, allied health professional (AHP) and medical colleagues.

Main duties of the job

• Rotation between partaking in the TIA service, Emergency Department Stroke care and ward cover.
• Facilitate TIA clinic appointments
• Assessing potential stroke patients in the Emergency Department who may be appropriate for time critical interventions, guiding the need for more advanced imaging as required. 
• Participate in the Integrated Stroke Delivery Network (ISDN) TIA workforce to ensure the Trust is working in line with local hospitals when delivering TIA care to patients.
• Act in the absence of the Stroke Nurse Consultant to answer questions which may be received from discharged patients, external agencies or GP surgeries.
• Provide education, advice and support to junior doctors regarding stroke/TIA presentation and the management thereof.
• Support the stroke outreach service by providing education, advice or help where required, in the absence of the Stroke Outreach Lead.
•  Provide advice and information to Stroke Data Administrator to ensure accurate input of data to Stroke Sentinel National Audit Programme (SSNAP).
• Act as a source of expertise and knowledge for patients, carers and health professionals as well as other agencies involved in stroke services.
•  Participate in the daily multidisciplinary discussions (board round) to ensure patients are receiving appropriate evidence-based care.
• Provide clinical cover to stroke according to patient and staffing needs. This can include cover of post take ward rounds in association with the Stroke Consultant on call.
