Job overview

We are looking for dynamic, enthusiastic, professional individual to come and work with our amazing  District Nursing Teams across Poole Bay area to support the health of our community.

We have a part time maternity cover post in the Shore District Nursing team working from Parkstone Towers Medical Centre

As a community staff nurse you will be providing nursing care to patient in their own home. We provide a wide and varied level of care to patient in their home. This means we have highly skilled workforce, successful candidates will be supported with upskilling and training. 

There will be an expectation for the successful candidates to work weekends, late shifts and bank holidays pro rata. Our late shift covers until 8pm. 

We would welcome applications from newly qualified nurses and we provide a very comprehensive and fully supported Preceptorship Programme for newly qualified healthcare professionals.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
 

Main duties of the job

To provide a high quality person centred approach to care delivery which always considers people's safety, privacy and dignity. 

To undertake assessment of patients / service users care needs and develop a programmes of care as part of the wider multi-disciplinary care team. 

To provide advice to patients/ service users and carers regarding their care. To plan, implement and review health improvement programmes in a range of settings. 

To provide supervision, mentorship, and clinical support to students and junior staff in the team.  

To assess the needs of patients in a variety of settings and formulate a plan of care.  This may involve liaison with carers, other health professionals, GP’s, consultants, hospital nurses, social workers, hospice staff, specialist nurses, voluntary and statutory workers. 

To provide evidence based nursing care to a defined patient caseload with the support of the District Nurse.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only.  Please obtain permission to be released on a secondment before submitting an application.
