Job overview

The SPA team is looking for Health and Social Care Co-ordinators to join our developing team.

This is an exciting and developing role. It is pivotal to bring services together in order to provide seamless care for frail and vulnerable individuals.

Working closely with our on site multi disciplinary team, social colleagues and acute hospital partners you will work to help to reduce the
occurrence of ill health, hospital admissions and ensure timely and safe discharge of patients into community services.

Main duties of the job

This role is seen as a critical and evolving post to support the SPA Multi-Disciplinary Teams (MDTs) across Dorset to deliver Home First in an effective, co-ordinated manner for those individuals identified as being no longer requiring to reside in an acute or community hospital bed.

To work closely with acute and locality hub-based health and social care teams to:

·      Ensure timely processing of system referrals and attending meetings to coordinate care across the system

·       Provide effective co-ordination of the multi-disciplinary team (MDT) meetings and follow through on agreed actions

·       To access and monitor and alert relevant professionals where safeguarding risks are identified.

·       Support system partners with information and attend meetings to provide updates and take further actions as agreed

·       To have access to Primary, Community and Social Care systems to facilitate effective multi-agency communication.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Knowledge

Essential criteria
  • NVQ Level 3 qualification or equivalent experience
  • Evidence of consistent pattern of learning from education, training and experience

Job Specific

Essential criteria
  • Experience of working under own direction
  • Experience in a customer focussed environment
  • Experience of Chairing, planning and organising complex meetings/ agendas
Desirable criteria
  • Experience of supporting service improvement
