This is an exciting pioneering role for carers seeking opportunities for professional development in Health & Social Care.

We are looking for part-time Band 3 Community Support Workers to support holistic and personal care for people at the end of their lives, ensuring the best possible outcomes for people in our care.

This role involves providing domiciliary care for a wide variety of patients, offering a short-term service whilst a longer-term package of care is being secured.

Patients will be supported in their home environment to prevent avoidable hospital admissions, facilitate earlier hospital discharges as well as in-reaching to the wards.

In exchange for your enthusiasm and commitment, you will be provided with a comprehensive introduction into care with training and ongoing support to give you the skills you’ll need to be flexible in your approach and confident to work alone.

 

 

Main duties of the job

 Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

The main duties are to work with District and Community nurses, who will lead the team and delegate wellbeing care plans, to provide support for patients having palliative care at home. this could include personal care, help to move, pressure area care, minor wound care, prompting medication, help with eating and drinking, ensuring a clean and safe environment, promoting independence and health and wellbeing.

Emotional support and mental health is also considered; signposting to appropriate services for palliative and end of life care. The team work  proactively and independently as well as reporting back and communicating effectively with the multidisciplinary team. The team also participates in assessment of people’s needs, contribute to preventing avoidable admissions and supporting earlier discharges from Acute and Community Hospitals for End of Life Patients.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Knowledge, skills and training

Essential criteria
  • A care cert already undertaken or a willingness to undertake the Care Certificate
  • Willing to professionally develop and undertake further development/ training, as identified to meet the needs of the service
  • Excellent communication, interpersonal, organisational and decision-making skills
  • Car owner/ driver, and be prepared to travel extensively in rural areas
  • Skills to adapt to and manage occasional challenging situations
  • Demonstrate skills in written and spoken English, adequate to enable the post holder to carry out the role effectively
Desirable criteria
  • experience in holistic palliative care

Job Specific experience

Essential criteria
  • Healthcare assistant experience or experience of working in a healthcare environment
  • experience of working in community and people's homes
  • ability to teach families and carers how to provide/participate in personal care

  • Recent previous experience of working within a customer focused environment

  • IT literacy in order to be able to enter patient-related data into identified electronic record systems
