Job overview

The Integrated Community Rehabilitation Team is a multi-disciplinary team offering therapy to a wide range of people within their home environments to prevent avoidable hospital admissions and facilitate earlier hospital discharge. It is essential that you have experience of rehabilitation or a caring role, preferably in the community settings, as well as having good verbal, written and team working skills. You will need to be flexible in your approach and confident to work alone. Basic IT skills and NVQ level 2/3 would be advantageous.

Responsibilities will require you to work mainly under the close supervision of the clinicians within the team, but at times also to work autonomously. 

In return for your enthusiasm and commitment, we can provide an opportunity for you to develop your clinical skills and experience in a supportive team, with regular supervision and appraisals. You will be required to achieve a certain competency level which we will give you support and training to help you reach. Opportunities for further training may be available through the Trusts Training Department.

Hours covered by the service are 08:00-20:00, 7 days per week on a rota basis with weekend enhanced pay! Being a driver is essential for this role.

Main duties of the job

Providing rehab based care to people in their own home or on the ward, this will include undertaking personal care tasks such as helping someone wash and dress. 

Completing supervised assessments of mobility, equipment and function

Monitoring deteriorating patients with support from clinicians 

Working with other Rehab Assistants to provide the service across 7 days 

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

KNOWLEDGE, SKILLS AND TRAINING

Essential criteria
  • NVQ care level 2/3 or willing to work towards
  • Skills to manage challenging situations
  • Good organisational skills/decision making

JOB SPECIFIC EXPERIENCE

Essential criteria
  • Experience working in Health or social care setting
  • Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
  • Ability to prioritise and organise workload effectively
Desirable criteria
  • Experience of working in the community
  • Previous experience of rehab of elderly/adults

FINANCE/RESOURCES

Essential criteria
  • Able to effectively manage available resources in the pursuit of quality service provision ensuring a safe environment

INFORMATION TECHNOLOGY/RESOURCES

Desirable criteria
  • Able to evaluate data and produce reports using Microsoft Excel and Word
  • Experience of using electronic patient / service user record systems

PERSONAL QUALITIES/ATTRIBUTES

  • Evidence of demonstrating the Trust’s values and behaviours.
